- Ensure that all personnel are fully aware of the Break of Containment Procedure/ Guidance Notes and all documentation is completed i.e. Break of Containment checklist etc.
Under no circumstances should the work commence until all present are 100% satisfied with the proposed scope and the safety precautions to be implemented
- Utilise only the correct equipment for the work being carried out.
- When opening a flange / joint, never stand directly alongside or below the joint, in case there are any leakages.
If there are any signs of pressure in the system, STOP immediately and re-tighten the flange to prevent any possible leakage and notify the supervisor asap.
- Do not trust gauges, but prove line is de-pressurised by physically opening drains, vents etc.
